Jeffrey M. Bullard, a native Texan, earned his Bachelor of Science Degree from the University of Wyoming. “From the time I was very young,” states Dr. Bullard, “there was simply something in me that told me I was supposed to do this.” Dr. Bullard earned his Doctor of Medicine Degree in 1993 from Creighton University’s School of Medicine in Omaha, Nebraska.
During his senior year, while serving as Class President, he was asked to serve as a member of the Executive Committee for the medical school and received the national Upjohn Achievement Award. In 1996 Dr. Bullard completed his Family Practice Residency in Norfolk, Virginia, where he served as Chief Resident. While there he received the Isadore Marman Award for outstanding resident. He has been board certified in Family Medicine since 1996.
Dr. Bullard established MaxHealth Family Medicine in 1998 with a simple but firm practice philosophy: his patients must know that their medical issues are important to him. In addition, they must feel their time, comfort and confidence are just as important. Because of this vision, he continues to demand that MaxHealth provide cutting edge technology and medical care. “We must keep abreast of the newest medical information and provide that level of medical service. That is the kind of treatment our patients deserve.
” Dr. Bullard was a pioneer in changing the old accepted norm of long wait times and impersonal medical care. He structured schedules that allowed for both scheduled appointments and walk-in appointments every day throughout the day, now coined the ‘open access’ model. When his practice opened in 1998, it was an entirely electronic record system — one of only 3 in the DFW area at that time. Another essential he insists upon is that his patients receive their medical care in a warm, welcoming environment. In 1998, as a result of his innovative thinking, he was featured in the Dallas-Fort Worth Healthcare Journal in recognition of his progressive thinking.
In 2007, Dr Bullard partnered with Leigh Richardson to form the BrainInstitute. “Now we are able to take our patient evaluations to the next level and provide, in addition to their routine health physicals, brain physicals.” Using the latest technology, patients can receive help addressing their mental and brain health needs. Over the years Dr. Bullard has been asked to teach throughout the state on topics ranging from the diagnosis and treatment of depression to the management of hypertension. For his absolute and unwavering persistence on innovation and customer respect, he was honored by being voted a “Top Doc” of Fort Worth in 2006 by his peers and has been featured on the television show Top Docs of DFW. Dr. Bullard, his wife Molly, and their two sons, Max and Zack, enjoy exercising, skiing and snowboarding, boating and wake-boarding, SCUBA-diving, hiking, and other family activities.

Dr. John Moorhead grew up surrounded by family members helping others through the practice of medicine. His maternal grandfather was one of the first African American physicians in South Florida and practiced general medicine well into his nineties. Dr. Moorhead’s father was an OB/GYN practicing in Atlanta for over 30 years.
His uncle was a General Practitioner in the US Virgin Islands. He also has an older brother and sister who are both practicing psychiatrists. With so much exposure to such an interesting and complex field, he was able to determine at an early age the career path that he wanted to take as an adult.
While in high school, Dr. Moorhead’s interests in science ignited winning ideas for both local and state sponsored science fair competitions. After winning a national award for a project that measured the effects of various medications creating insulin resistance in rats with type II diabetes, his decision to follow in his family’s footsteps was sealed. Dr. Moorhead got a jumpstart on his medical career by taking freshmen college courses during his senior year of high school. He was then accepted into Morehouse College where he earned a Bachelor of Science Degree.
Sponsored by the Navy, he then graduated from Morehouse College with his Doctor of Medicine Degree. His internship was completed at Balboa Naval Hospital in San Diego, California. PA Kelly Gonzales’ interest in overall health and wellness began as she was growing up in Houston, Texas. Even though she always had an interest in medicine and patient care, she discovered she had a great propensity for numbers and enrolled at the University of Texas at Austin to begin the coursework for a degree in Finance. She later graduated with Honors with a Bachelor of Business Administration in Finance.
While working as a financial analyst in a large oil company, a close relative developed breast cancer. In answer to this misfortune, Kelly began volunteering at the MD Anderson Cancer Center and the Make-A-Wish Foundation. As she became more and more active in these organizations, she again felt the draw of medicine.
Kelly soon began the preparatory academic work towards earning a Physician’s Assistant degree. During this time, she worked both at MD Anderson and The Texas Heart Institute in the research department helping develop artificial hearts and valve replacements. She was soon accepted into the University of North Texas Health Science Center. While earning her degree, she met her husband, a fellow PA student. They both graduated in 2000 and later married.
PA Gonzales began practicing in an urgent care setting, but quickly found she missed following up with her patients and being able to address their overall health, so she made the shift into Family Medicine in 2003. At MaxHealth, she is able to address her patients’ acute needs and also serve as their life coach, helping them set and achieve their personal health goals. Kelly enjoys developing long term relationships with her patients so she can educate and support them, giving them the tools and knowledge they need to take an active role in both their mental and physical health. She also enjoys the contribution she makes teaching PA students at her alma mater.
Kelly spends her free time with her husband David and their two sons, Rocco and Lucas, family and friends. Whether traveling, exercising or just spending time with her family, PA Gonzales embraces the same standards she holds her patients to – taking an active role in both her and her family’s health to stay healthy physically and mentally.
PA Stephanie West’s interests have always centered on health and medicine. As a child, she kept her family’s Health Reference Book on her nightstand and read it regularly. As a teenager, she exchanged the family reference for an actual medical textbook which was always at her fingertips.
As she learned about the human body and how each of it’s different systems interacted with one another, her interest and amazement intensified. At a pivotal time in her life, a member of her family became ill and passed away. Stephanie watched in awe as her mother was able to read and understand the medical chart and autopsy report. At this moment, her desire to enter into the field of medicine gained its momentum.
PA West first obtained a degree in Environmental Health Science and subsequently achieved further master level education in similar studies. After meeting her husband and having two children, she decided the time was right to pursue her call to medicine and was accepted into a highly competitive Masters in Physician Assistant Studies Program at University of North Texas Health Science Center with her first application. She graduated three years later with Honors. Stephanie immediately went to work for the Dallas VA Medical Center in the Internal Medicine Department. The position was incredibly rewarding in that it involved dealing with very complicated patients and provided an invaluable learning experience. However, the two hour commute led her to find a medical practice closer to home. In 2006, she discovered MaxHealth, a flourishing family practice, where she has been practicing ever since.
As a Physician Assistant, Stephanie has incorporated her Environmental Health background into her medical practice. She has found that medicine is not just about pharmaceuticals, but involves a great deal more. Genetics, lifestyle, and even attitude can make a difference in outcomes. Each person presents their own unique set of circumstances, challenges and achievements; every person is unique and individual. As a provider, PA West has learned to use this holistic approach with her patients and feels that is part of what makes medicine so interesting. She truly enjoys working with her patients in partnership to help them achieve their goals and finds sharing in these accomplishments incredibly rewarding.
Outside of her time with patients, PA West looks forward to family time and spending time outdoors, whether walking, often with the family’s dogs, running or playing tennis. She also has an avid interest in art and enjoys time spent painting.
