Jeffrey M. Bullard, MD

Jeffrey M. Bullard, MD is a native Texan, born in Longview. He received his Bachelor of Science from the University of Wyoming and then attended Creighton University in Omaha where he earned his Doctor of Medicine Degree in 1993. During his senior year, he served as Class President and received the national UpJohn Award for outstanding medical student. In 1996, he completed his Family Practice Residency in Norfolk, VA., where he served as Chief Resident and received the Isadore Marman Award for outstanding resident. Dr. Bullard was also voted “Top Doc 2006 by his peers and is board certified in Family Practice. He enjoys partnering with members of the community to increase their health awareness. Dr. Bullard, his wife, Molly and their two sons, Max and Zack enjoy exercising, skiing, boating, SCUBA and family activities. Molly earned her Bachelor of Science degree from the University of Nebraska and works part time for MaxHealth.

Sayeh Barzin, DO

Sayeh Barzin, DO grew up in the Dallas/Fort Worth Metroplex. She received her Bachelor of Science in Biology from The University of Texas at Arlington and graduated Magna Cum Laude. While in college, she received many honors including the Outstanding Scholastic Achievement Award, the College of Science Academic Excellence Award, and was on the National Dean’s List. She attended medical school at The University of North Texas Health Science Center at Fort Worth. Dr. Barzin completed her residency at The University of Texas Southwestern Medical School/Methodist Hospital in Dallas where she served as Chief Resident in her program. Dr. Barzin has a special interest in women’s health, for which she received additional training during her residency. She enjoys traveling and spending time with family and friends, being involved in community service activities, and training medical students. She looks forward to establishing a long-term relationship with her patients and providing the best medical care possible.

John A. Moorhead, MD

John A. Moorhead, MD is a third generation physician and grew up in Atlanta, GA. He attended Morehouse College, where he earned his Bachelor of Science degree. He then obtained his Doctor of Medicine degree from Morehouse School of Medicine, sponsored by the US Navy. After finishing his internship, he served a tour of duty as a Naval Flight Surgeon. In 1996, he completed his Family Practice Residency at Norfolk General Hospital in Virginia. Dr. Moorhead and his wife Hilary enjoy playing soccer, biking, running 10K races and traveling. John also likes playing polo and is an avid fan of MLS soccer and the Dallas Mavericks. He is also a patron of the theater and the opera, as well as enjoys cooking at home and fine wines. He looks forward to building long-term relationships with his patients while providing quality healthcare.

Kelly A. Gonzales, PA-C

Kelly A. Gonzales, PA-C was raised in Houston, TX. She graduated with Honors from The University of Texas at Austin in 1993 with a BBA in Finance. While working in Houston as a financial analyst, Kelly volunteered at MD Anderson Cancer Center and Make a Wish Foundation, soon realizing she had a strong propensity for medicine. She quickly began the preparatory academic work towards a physician assistant degree. PA Gonzales graduated from UNT Health Science Center in Fort Worth and became licensed by the NCCPA in 2000. She enjoys spending time with her husband David, and their sons, Rocco and Lucas, close friends and traveling. PA Gonzales has a strong interest in preventive health and looks forward to providing patients with the tools and resources to better their lives physically and mentally.

Ann Sullivan, PA-C

Ann Sullivan, PA-C, MPAS grew up in Paris, IL. She attended Butler University in Indianapolis where she earned a BS in Biology. PA Sullivan graduated Cum Laude from Butler University/Methodist Hospital PA program in 2001 and became certified by the NCCPA. She moved to Texas and worked in vein harvesting and cardiothoracic surgery in the Fort Worth area before coming to MaxHealth in 2003. She loves caring for others and giving back to her community through volunteer work. Ann has a heart for mission work and has recently worked in Honduras, Liberia and Kenya. PA Sullivan finished her Master's in November 2009 through the University of Nebraska Medical Center. She enjoys spending time with her husband, Bill, as well as exercising, traveling and being active in her church. Ann's goal is to consistently provide excellent medical care while bringing joy and laughter into the lives of her patients.

Stephanie West, PA-C, MPAS

Stephanie West, PA-C, MPAS graduated from UNT Health Science Center in Fort Worth with a Master’s Degree in Physician Assistant Studies. She received Honors throughout her schooling. She grew up in the Dallas/Ft. Worth and and enjoys painting and playing tennis. Her favorite thing to do is spend time with her husband, Ted and their two children, Crystal and Hunter. She loves taking care of patients and truly enjoys the medical field. PA West is excited to serve this community and looks forward to providing excellent care for all her patients.

Leigh Richardson, BCIAC Associate Fellow

Leigh completed her didactic training for neurofeedback at the University of North Texas. She completed her internship with a nationally recognized neurologist and pioneer in the field of neurotherapy, Dr. Jonathan Walker, M.D., who has studied, researched and taught for 35 years. While working with Dr. Walker as a neurotherapist, Leigh received her BCIA, EEG certification as well. She is very passionate about the benefits of neurotherapy and is excited about using neurofeedback to achieve positive results in patients.

Jaime Ellis, Practice Manager

Jaime Ellis is originally from Michigan, but relocated to Texas with her family in 1991. While attending the University of Texas at Arlington, she began working at MaxHealth as a part-time receptionist. After working in various positions throughout MaxHealth and graduating in 2001 with a Bachelor of Business Administration, specializing in Management, she was promoted to the Practice Manager. Jaime ensures that MaxHealth functions at a high level of efficiency and is available for any comments, questions or concerns from our patients. Jaime enjoys reading, baking and spending time with friends, her husband, Robby and their three daughters, Kylie, Sydney and Lauryn.
