Patient Rights
- Have all of your medical records kept private
- Help your provider make decisions about your healthcare
- Discuss your condition and all care alternatives with your provider, including potential risks and benefits
- Choose your provider’s office
- Get up-to-date information about your provider and the hospital they admit to
- Be referred to a specialist who is experienced in treating your illness
- Receive a prompt reply when you ask questions or request information
- Suggest changes in our policies or services
- Call 911 in a situation that may be life-threatening
Patient Responsibilities
- Help your providers make decisions about your healthcare
- Tell your provider if you do not understand the treatment you receive and ask if you do not understand how to care for your condition
- Give correct and complete information to your provider and their staff
- Follow the directions and advice you and your provider agree upon
- Tell your provider immediately when you have unexpected problems or symptoms
- Consult with your provider for referrals to non-emergency specialists or hospital care
- See the specialists your provider refers you to
- Show your insurance card and driver‘s license prior to seeing a provider
- Read and understand your insurance plan and benefits
- Pay all applicable copays, coinsurance and deductibles for which you are responsible
