Request your records online with Healthmark

Requesting medical records can be confusing. HealthMark Group is here to help! As our release of information partner, HealthMark is here to streamline the process and get you your records faster and more efficiently.

How do you request records?

Requests may be submitted electronically through HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Your medical record request will be processed, and a notification will be sent via mail or email once complete and available for download.

How long does it take to process your requests?

Your record requests are generally processed within 8 business hours. What does that mean? One or two business days after you submit your request, your records will be delivered electronically – right to your inbox.

Do you have to remember another username and password?

Nope! HealthMark’s Request Manager does not require usernames or passwords for logging in. Simple email verification and secure links do the trick – getting you your records quickly, efficiently and securely – no password required.

Other ways to request your records

For the fastest, easiest access to your health information, use our online Patient Portal. Please be aware that the portal does not contain your full medical record. For example, your visit notes and records we receive from other providers will not be available on the Portal.

If you need copies of information that is unavailable on our Portal, follow the instructions above to request records through HealthMark. You can also fill out our paper request form, located on our website under Resources on the Patient Forms page, and submit your request, in person or by fax or mail:

Your records will be released as specified by you in the Authorization form. Once processed, you will receive a notification via email or standard USPS delivery with instructions on how to retrieve your records. To expedite the delivery of your records, please provide your email address on your authorization.

Check the status of your request

To check the status of a previously submitted request please contact HealthMark Group directly by phone at 800-659-4035 or by email at status@healthmark-group.com.

Outside Requestors

Providers, STAT and urgent requests for records should be directly faxed to 817-391-1070 for processing. Sending these requests to HealthMark will cause the request to be delayed. If you are sending records to us, these should also be sent by fax to 1-877-878-7146—not to Healthmark.

Payers and institutional requestors, please use Healthmark Group’s Medrelease tool to request copies.
