YOUR PRIVACY is very important to us. Please be sure your HIPAA forms on file are up-to-date and accurate. They detail to whom we can release your private health information, who can pick up things for you, and with whom we can discuss your healthcare. See a receptionist if you have not filled out these forms or if you need to make changes.

Patient Rights

  • Have all of your medical records kept private

  • Help your provider make decisions about your healthcare

  • Discuss your condition and all care alternatives with your provider, including potential risks and benefits

  • Choose your provider’s office

  • Get up-to-date information about your provider and the hospital they admit to

  • Be referred to a specialist who is experienced in treating your illness

  • Receive a prompt reply when you ask questions or request information

  • Suggest changes in our policies or services

  • Call 911 in a situation that may be life-threatening

Patient Responsibilities

  • Help your providers make decisions about your healthcare

  • Tell your provider if you do not understand the treatment you receive and ask if you do not understand how to care for your condition

  • Give correct and complete information to your provider and their staff

  • Follow the directions and advice you and your provider agree upon

  • Tell your provider immediately when you have unexpected problems or symptoms

  • Consult with your provider for referrals to non-emergency specialists or hospital care

  • See the specialists your provider refers you to

  • Show your insurance card and driver‘s license prior to seeing a provider

  • Read and understand your insurance plan and benefits

  • Pay all applicable copays, coinsurance and deductibles for which you are responsible
